
If you need to enroll a student, please use the enrollment packet linked below. We also need a copy of the student's birth certificate, social security card, and two proofs of residency (including at least 1 form of government issued ID - ex. driver's license) for the parent/guardian.
If the student enrolling is in 7th or 8th grade, we need a current copy of the student's immunization record.
If the student or family has any related legal documents (ex. custody, guardianship, adoption, foster/student placement, etc.), please provide a copy to the school.
If the student is transferring from another school and does not live in our school's district, there is a student transfer form that needs to be filled out and signed by our principal.
If the student will be taking any medication at school, there is a Medication Authorization form that will need to be filled out and signed by the student's doctor.
